How to Submit Your Request/Form:
Printed or saved forms must be submitted to the Enrollment Services Office in Building 17 on the Lakewood Campus.

- Email requests must be sent from the preferred email address listed on file.Ìý
- Drop off your completed form at the Enrollment Services Office with a valid photo ID.
